Notes:
Food Vendors are responsible for providing the city with current Food Handlers Permit verifications.
Deadline for proof of Food Handlers Permit verification and booth application with payment of $40.00 will be July 15, 2011.

All Vendors are required to obtain a Special Event License from the Utah State Tax Commission at the park on the day of the event.

Fee will be waived for Registered Non-Profit Organizations (LIMIT 1 BOOTH)

Booth times are from 10 am – 10 pm Saturday only. Set up time for booths are 8:00 am on July 30th.

PARK ADDRESS: 1350 N Hwy 89, Harrisville

Harrisville City reserves the right to ban the sale of any inappropriate material in the Harrisville City Park during Heritage Days 2011, on July 30, 2011.

ELECTRICITY IS PROVIDED FOR FOOD VENDORS AND A LIMITED NUMBER OF REGULAR VENDOR BOOTHS
